What is a functional assessment?
A functional assessment considers what a person can do, where difficulty occurs and how the injury affects everyday roles. It connects clinical information with observable function so the referrer receives a practical picture of current capacity, barriers and support needs.
Physical injury functional assessment
For a physical injury, the assessment may consider mobility, strength, tolerance, pain behaviour, transfers, task performance, access, safety and participation at home or in the community.
Psychological injury functional assessment
For a psychological injury, the assessment may consider routine, concentration, motivation, emotional regulation, social participation, confidence, task initiation and the effect of symptoms on everyday activity. The scope is tailored to the referral question and does not replace psychiatric diagnosis or treatment.

- Current functional abilities and limitations
- Barriers affecting activity and participation
- Safety, support or environmental considerations
- Practical recommendations linked to the referral question
- Further assessment or rehabilitation needs where indicated
